A new study warns that healthcare systems worldwide are moving into AI-assisted medicine faster than their ethical and regulatory frameworks can adapt. The analysis shows that even jurisdictions with relatively advanced governance systems still face unresolved questions over patient rights, bias, privacy, liability, human oversight, and the risk that clinicians may become too dependent on automated recommendations.

The study, titled “Guiding Policymakers Toward Better AI Ethics Integration in Healthcare Regulation—Lessons from Singapore,” was published in the Journal of Clinical Medicine. Using Singapore as a detailed example, the paper claims that the global challenge is not simply whether countries have AI rules, but whether those rules are integrated with medical ethics, professional duties, patient protections, and the realities of clinical care.

Healthcare AI regulation is expanding, but ethical integration remains weak

AI systems are already being used or tested for screening, triage, diagnostic support, referral management, image analysis, risk prediction, treatment planning, and workflow optimization. Governments have responded with new strategies, guidance documents, risk classifications, and medical device rules. However, the study contends that this growing body of regulation still leaves a central problem unresolved: many frameworks remain fragmented.

The researchers use Singapore to show how this fragmentation can appear even in a well-organized regulatory environment. Singapore has developed guidance for AI in healthcare, software medical devices, telehealth products, and emerging health technologies. It also has professional ethical codes covering doctors, nurses, dentists, pharmacists, midwives, and allied health professionals. The problem, according to the study, is that these documents do not always clearly explain how AI-specific responsibilities connect with existing clinical duties.

That problem is likely to be familiar to other countries. Healthcare AI sits at the intersection of medicine, data governance, product safety, digital infrastructure, professional ethics, and patient rights. A rule written for software developers may not tell clinicians how to discuss AI use with patients. A professional ethics code may protect confidentiality but say little about machine learning models trained on de-identified data. A medical device approval process may assess technical performance but give less guidance on cultural sensitivity, deskilling, or patient autonomy.

The study maps Singapore’s rules against nine major risks in medical AI: effectiveness and reliability, fairness and discrimination, privacy and confidentiality, machine paternalism, value pluralism, responsibility, trust, explanation and justification, and the danger of dehumanization or professional deskilling. These risks are not limited to Singapore. They are core governance problems for any country adopting AI in healthcare.

The findings suggest that broad ethical principles are now common across many AI frameworks. Governments often refer to fairness, transparency, safety, accountability, reliability, and patient-centered design. But the harder task is translating those principles into clear operational duties. Health systems need to know who is responsible for checking bias, who decides whether an AI tool is safe enough to deploy, when clinicians should override AI outputs, whether patients should be told about AI use, and how harms should be investigated.

Singapore’s framework is generally aligned with global standards, including approaches seen in the United States, European Union, United Kingdom, Australia, China, Brazil, and Japan. But the researchers find that alignment does not automatically create practical clarity. A system can contain many relevant documents and still leave gaps between policy, clinical practice, and patient experience.

One of the most important gaps is patient involvement. Many healthcare AI frameworks describe systems as patient-centered, but the study argues that this phrase can become vague unless regulators explain how patient voices should influence development and deployment. Patients should not be consulted only after an AI system is already designed. Their perspectives should help shape what the tool optimizes for, what outcomes matter, what risks are acceptable, and how feedback should be handled after deployment.

This is especially important because healthcare AI can affect deeply personal decisions. AI systems may influence referrals, treatment priorities, clinical risk scores, or recommendations in areas such as chronic illness, fertility care, mental health, disability support, and end-of-life care. In these settings, patient values can be as important as technical accuracy.

The study warns that patient engagement must also be representative. If developers and regulators consult only digitally fluent, health-literate, or well-connected patient groups, they may miss the concerns of elderly patients, people with disabilities, lower-income communities, migrants, minority language groups, and patients with limited access to care. That risk applies worldwide, especially in countries where digital health tools are expanding faster than public participation mechanisms.

Cultural sensitivity is another global concern. Singapore’s diverse population makes the issue visible, but the principle applies to all health systems. AI tools trained or validated in one setting may not work ethically or effectively in another if they ignore local values, languages, family roles, health beliefs, and social conditions. A technically strong system may still fail if it does not fit the community it serves.

The study also challenges the assumption that explainability should always be the central standard for ethical AI. Explainability matters, especially for auditing, error review, legal accountability, and high-risk decisions. But the researchers argue that justifiability may sometimes be more important. In healthcare, a tool may be ethically acceptable if its use can be justified through evidence, patient benefit, risk management, and respect for patient values, even when its internal reasoning cannot be fully explained.

This distinction matters for global regulators. A rigid demand for full explainability may exclude useful systems, while weak explainability standards may allow unsafe or unaccountable tools. The study suggests that regulators need a more balanced approach: require transparency and explanation where they are necessary, but also ask whether the system’s use can be justified to clinicians and patients in the specific care context.

Bias, privacy, and liability are becoming global pressure points

Bias remains one of the hardest problems in healthcare AI because medical data reflects the inequalities of real healthcare systems. If some communities have historically received less care, later diagnoses, fewer referrals, or lower-quality documentation, those patterns can become embedded in machine learning models. AI may then reproduce unequal treatment while appearing neutral.

The study uses Singapore to examine how this problem can arise in practice, but the issue is global. AI systems trained on incomplete or unequal datasets may underperform for certain ethnic groups, age groups, genders, socioeconomic classes, or patients with rare conditions. In countries with large regional, racial, caste, language, or income-based disparities, the risk may be even greater.

The researchers argue that regulators need clearer rules for biased AI. Total elimination of bias may not be possible in all systems. The ethical question is therefore not only whether bias exists, but whether the tool improves or worsens real-world care compared with existing practice. A biased AI system may still produce better outcomes than a biased human system, but it may also deepen inequality if deployed without monitoring.

This creates a difficult trade-off between utility and fairness. A tool might improve early detection overall while still producing uneven results across groups. Regulators need practical methods to decide whether such a tool should be approved, limited, redesigned, monitored, or withdrawn. The study argues that healthcare systems need ethical proportionality tools that help decision-makers weigh benefit, harm, fairness, privacy, access, and public trust.

The privacy challenge is also changing. Traditional privacy frameworks often focus on identifiable personal data, but AI can infer sensitive information from data that appears anonymous. Medical images, clinical notes, genetic data, wearable device data, and hospital records may reveal patterns about race, disease risk, social background, or identity even when obvious identifiers are removed.

The study warns that this weakens the boundary between identified and de-identified data. Singapore is cited as an example where existing data protection approaches may not fully address re-identification risks created by AI. But the broader lesson applies to any country relying on older privacy concepts in a machine learning environment.

Healthcare systems may need stronger technical safeguards, stricter rules for data sharing, and clearer limits on AI-enabled re-identification or surveillance. Contractual safeguards alone may not be enough when powerful models can extract sensitive signals from large datasets. This is particularly important as hospitals, governments, insurers, and private technology companies seek to use health data for AI development.

Liability is another unresolved global pressure point. When an AI-assisted decision harms a patient, responsibility may be spread across many actors. The clinician may have used the recommendation, the hospital may have bought and deployed the system, the developer may have designed the model, the regulator may have approved it, and the vendor may have updated it after deployment.

This distribution of responsibility can leave patients without clear answers and clinicians without clear protection. A doctor may be blamed for following AI advice in one case and for ignoring it in another. A hospital may assume approval is enough, while a developer may argue that misuse occurred at the clinical site. The study argues that governments must clarify responsibility before AI becomes deeply embedded in routine care.

The challenge becomes more serious with adaptive AI systems. Some models may change over time as they encounter new data, different populations, or updated workflows. A tool that performed well during validation may behave differently after deployment. That means regulatory approval cannot be treated as a one-time checkpoint. Countries need lifecycle oversight, post-market monitoring, performance audits, and clear triggers for review.

The study also recommends public registries of approved AI and machine learning-enabled medical devices. Singapore has a broader medical device register, but the researchers argue that a dedicated AI/ML list would improve transparency. For other countries, the same idea could help clinicians, hospitals, patients, and developers see which tools have been reviewed, what they are approved for, and where they can be used.

Such registries could also support public trust. Healthcare AI is difficult for patients to evaluate directly. A clear public record of approved systems can show that tools have passed some form of regulatory review, while also making it easier to track updates, safety issues, and evidence standards.

Professional ethical codes also need revision. Many medical ethics frameworks were written before AI became part of clinical decision-making. They address confidentiality, consent, professional judgment, and patient welfare, but may not explain how these duties apply when AI produces a diagnosis, risk score, or treatment suggestion. The study argues that healthcare professionals need direct guidance on AI-assisted care.

This includes guidance on whether AI use should be disclosed to patients, how clinicians should explain AI-supported recommendations, when patients should be allowed to seek human review, and how professionals should respond when AI outputs conflict with their judgment. These questions cannot be answered by technical regulation alone. They belong at the heart of the doctor-patient relationship.

Human oversight must remain meaningful as automation grows

The study warns that healthcare AI can reshape professional behavior in two opposite ways. Some clinicians may over-rely on AI outputs, a problem known as automation bias. Others may reject AI recommendations because of distrust or fear, a problem known as technology bias. Both can damage patient care.

Automation bias may cause clinicians to miss warning signs, ignore contradictory evidence, or accept machine-generated outputs too quickly. Technology bias may prevent patients from benefiting from accurate and well-validated tools. The researchers argue that health systems must train professionals to use AI critically, not passively and not defensively.

This is a global issue because many AI governance frameworks call for human oversight without defining what meaningful oversight requires. A human in the loop is not enough if the clinician does not understand the tool’s limits, lacks time to review its output, or has no authority to challenge it. Oversight must be built into clinical workflow, training, accountability systems, and institutional culture.

The study argues that different tools require different levels of oversight. Low-risk administrative tools may need less human review. High-risk diagnostic or treatment systems may require close supervision. Tools that directly affect patient autonomy, prognosis, or major treatment decisions may require stronger explanation and justification. Regulators should not treat all AI systems as if they carry the same risks.

Deskilling is another concern. If AI takes over repeated clinical tasks, professionals may lose important skills. This risk is already being debated in fields such as imaging, endoscopy, screening, and clinical decision support. Singapore is cited as an example where some institutions have considered AI-free periods to preserve professional competence. But the study stresses that evidence remains incomplete, and not all AI use will produce the same deskilling risk.

A better approach would identify which skills must be preserved, which tasks can be safely automated, and how clinicians should maintain competence in AI-supported environments. Some areas may require periodic unaided practice. Others may require audit-based review, simulation training, or structured continuing education. The goal is not to block AI, but to prevent professional judgment from weakening as automation expands.

The study also cautions against treating AI systems as moral agents. Trust should rest with the people and institutions responsible for developing, approving, deploying, and using AI. AI tools should be assessed for reliability, safety, performance, and clinical value. Patients should not be encouraged to believe that a system understands them, cares for them, or carries moral responsibility.

This concern is especially important as AI interfaces become more conversational and human-like. Patients may disclose sensitive information, accept recommendations, or form emotional dependence if a system appears empathetic or authoritative. Developers and healthcare institutions should avoid design choices that blur the line between technical assistance and human care.

On the whole, AI could help reduce delays, improve diagnosis, support clinicians, expand access, and make healthcare systems more efficient. But without stronger governance, it could also deepen disparities, weaken accountability, expose private health data, confuse patients, and erode clinical skills.
